What is a 1583 form?

The USPS Form 1583, also known as the Application for Delivery of Mail Through Agent, is used to authorize a commercial mail receiving agent (CMRA) to receive mail on behalf of a customer. The form has to have a duplicate for the agent to have in order for them to perform the tasks. It also needs to be notarized.
